What is PicoSure® tattoo removal?
At Dr. Avaliani’s Park Avenue clinic, PicoSure® laser tattoo removal is delivered with physician oversight, advanced pain management, and a meticulously curated approach to results. Every treatment plan is individualized. Every decision is intentional.
PicoSure® is the world’s first picosecond aesthetic laser, delivering ultra-short bursts of energy in trillionths of a second. Unlike traditional nanosecond lasers that rely on heat to break down ink, PicoSure® uses PressureWave™ technology to create a photomechanical impact that shatters ink particles into tiny fragments, small enough for your body’s immune system to naturally eliminate them.
How PicoSure® Tattoo Removal Works
PicoSure® tattoo removal follows a straightforward, proven protocol:
- Dr. Avaliani evaluates your tattoo, ink colors, depth, density, location, and skin type, to design the optimal removal plan
- Topical numbing cream is applied 20 minutes before treatment. Air cooling systems provide additional comfort during the session
- PicoSure® delivers ultra-short picosecond pulses that create a pressure wave, shattering ink particles into microscopic fragments
- Your immune system naturally absorbs and eliminates the shattered ink particles over the following 4-8 weeks
- Over 4-6 sessions (spaced 4-8 weeks apart), the tattoo progressively fades until 80-90% clearance is achieved
What We Can Remove
Full Tattoo Removal
Complete removal of unwanted tattoos on any body area. PicoSure®’s picosecond technology shatters all ink colors, including stubborn greens, blues, and reds, achieving 80-90% clearance in significantly fewer sessions than traditional lasers.
Selective Lightening
Targeted lightening of specific tattoo areas in preparation for a cover-up design. Dr. Avaliani can selectively fade portions of an existing tattoo while leaving other areas intact, giving your tattoo artist a better canvas for the new design.
Multicolor Tattoos
Tattoos with multiple ink colors that traditional lasers struggle to remove. PicoSure® is FDA-cleared to treat all ink colors on all skin types, including the notoriously difficult green, blue, purple, and red pigments.
Previously Treated Tattoos
Tattoos that have been partially treated with traditional Q-switched or Nd:YAG lasers but have stalled in progress. PicoSure®’s different mechanism of action can break through the plateau and continue fading ink that other lasers left behind.
Dark Skin Types
Patients with darker skin tones (Fitzpatrick IV-VI) who were previously told tattoo removal was too risky. PicoSure®’s pressure wave technology minimizes thermal damage, significantly reducing the risk of hyperpigmentation and scarring on all skin types.
Small & Large Tattoos
From small finger tattoos to full-sleeve and back pieces. Small tattoos can be treated in under 5 minutes per session. Larger tattoos may require longer sessions and injectable anesthesia for comfort. PicoSure® treats any body area including face, neck, chest, back, arms, and legs.
PicoSure® Technology Advantages
Why PicoSure® delivers superior tattoo removal results:
PressureWave™ Technology
PicoSure® delivers energy in picoseconds (trillionths of a second) rather than nanoseconds, creating a photomechanical pressure wave that shatters ink into microscopic particles. This mechanism targets ink more efficiently than heat-based approaches, achieving better clearance with fewer sessions and less risk of thermal damage to surrounding skin.
All Ink Colors Treated
Traditional Q-switched and Nd:YAG lasers are limited to black and dark blue inks. PicoSure® is FDA-cleared to treat all ink colors, including the notoriously stubborn green, blue, red, orange, and purple pigments that other lasers leave behind. This makes PicoSure® the most versatile tattoo removal laser available.
Fewer Sessions Needed
PicoSure® typically achieves 80-90% clearance in 4-6 sessions, compared to up to 20 sessions with traditional Q-switched lasers. The picosecond pulses break ink into smaller particles faster, allowing your immune system to clear them more efficiently between sessions. Sessions are spaced 4-8 weeks apart for optimal results.

Does It Hurt?
Your comfort options (we tailor this to you)
Most patients describe PicoSure® as feeling like a rubber band snapping against the skin, brief and very tolerable. Dr. Avaliani prioritizes your comfort at every step. Pro-Nox™ (laughing gas) is available before treatment for immediate relaxation with no needles required, topical numbing cream is applied 20 minutes prior, and air cooling provides additional comfort during the session. For larger tattoos, injectable anesthesia is available for maximum comfort.
Small tattoos can be treated in under 5 minutes, while larger pieces take longer. After treatment, you may notice temporary whitening (frosting) of the skin and mild swelling, which resolves within hours. There is minimal downtime, just avoid direct sun exposure on the treated area and follow simple aftercare instructions.

What Does PicoSure® Tattoo Removal Cost?
Pricing Information
No two tattoos are the same, and neither are our patients. PicoSure® tattoo removal cost depends on tattoo size, color complexity, ink density, location, and your goal (full removal vs. cover-up fading). Exact pricing requires an in-person evaluation. PicoSure® Tattoo Removal frequently asked questions
What is PicoSure® and how is it different from traditional tattoo removal lasers?
PicoSure® is the world’s first picosecond aesthetic laser, delivering energy in trillionths of a second rather than the billionths of a second used by traditional Q-switched lasers. This ultra-short pulse creates a PressureWave™ that shatters ink particles mechanically rather than thermally, resulting in better clearance with fewer sessions and less risk of scarring or skin damage.
How many sessions will I need?
Most patients achieve 80-90% clearance in 4-6 sessions, compared to up to 20 sessions with traditional Q-switched lasers. The exact number depends on your tattoo’s size, ink colors, density, depth, location, and your skin type. Sessions are spaced 4-8 weeks apart to allow your body to clear the shattered ink particles between treatments.
Can PicoSure® remove all tattoo colors?
Yes. PicoSure® is FDA-cleared to treat all ink colors on all skin types (Fitzpatrick I-VI). This includes the notoriously difficult green, blue, red, orange, and purple pigments that traditional Q-switched and Nd:YAG lasers often cannot effectively remove. PicoSure®’s pressure wave mechanism shatters all pigment types into particles small enough for immune clearance.
Does tattoo removal hurt?
Most patients describe the sensation as a rubber band snapping against the skin, brief and very tolerable. Topical numbing cream is applied 20 minutes before treatment, and air cooling systems provide additional comfort. For larger tattoos, injectable anesthesia is available. The actual treatment time for small tattoos is under 5 minutes.
Is there any downtime after PicoSure® treatment?
Minimal. You may notice temporary white frosting of the treated area and mild swelling immediately after treatment, which resolves within hours. Avoid direct sun exposure on the treated area and follow simple aftercare instructions. Most patients resume normal activities immediately. PicoSure®’s minimal thermal impact means significantly less downtime compared to traditional laser removal (which often requires 5-7 days of recovery).
Can PicoSure® treat darker skin types?
Yes. PicoSure® is FDA-cleared for all skin types, including Fitzpatrick IV-VI (darker skin tones). The picosecond pulse duration and pressure wave mechanism minimize thermal damage to surrounding tissue, significantly reducing the risk of hyperpigmentation, hypopigmentation, and scarring that traditional lasers pose for darker skin types.
Can I use PicoSure® to lighten a tattoo for a cover-up?
Absolutely. Selective lightening is one of PicoSure®’s most popular applications. Dr. Avaliani can target specific areas of your existing tattoo to fade them while leaving other areas intact, giving your tattoo artist a better canvas for the new design. Typically 1-3 sessions are needed for adequate lightening before a cover-up.
Will PicoSure® cause scarring?
The risk of scarring with PicoSure® is significantly lower than with traditional thermal lasers. The picosecond pulse duration delivers energy so quickly that it shatters ink mechanically rather than burning it, minimizing thermal damage to surrounding tissue. Most patients experience no scarring when proper aftercare instructions are followed.
What if my previous tattoo removal treatment stalled?
PicoSure® is an excellent option for patients whose tattoo removal has plateaued with traditional lasers. The different mechanism of action (pressure wave vs. thermal) can break through the plateau and continue fading ink that other lasers left behind. Many patients who switched to PicoSure® after stalling with Q-switched lasers see renewed progress.
What areas of the body can PicoSure® treat?
PicoSure® can treat tattoos on virtually any body area including the face, neck, chest, back, arms, hands, legs, feet, and fingers. Treatment time depends on the tattoo size: small tattoos take under 5 minutes, while larger pieces require longer sessions. Dr. Avaliani will assess your specific tattoo during your consultation.
